We collect nonpublic personal information about our clients from the following sources:

• Information we receive on applications or other forms, such as name, address, social security number, telephone number, income and occupation.
• Information about our clients’ transactions with us and our affiliates, such as claims and payment history.
• Information we receive from a consumer reporting agency, such as a credit bureau.

We do disclose nonpublic personal information only to the insurance companies that our agency represents as is allowed by Section 8(G) of Indiana Department of Insurance Rule 760 IAC 1-67. Otherwise, we do not disclose any nonpublic personal information about our clients or former clients to anyone, except as permitted by law.

Our practices regarding information confidentiality and security:

We restrict access to nonpublic personal information about our clients to those employees who need to know that information in order to provide products or services to our clients. We maintain physical, electronic and procedural safeguards that comply with applicable regulations to guard our clients’ nonpublic personal information.

Our policy regarding dispute resolution:

Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgement upon the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.

Reservation of the right to disclose information in unforeseen circumstances:

In connection with the potential sale or transfer of its interests, Hylant Group. and its affiliates reserves the right to sell or transfer our client’s information (including but not limited to our client’s address, name, age, sex, zip code, state and country of residency and other information that you provide through other communications) to a third party that (1) concentrates in business in a similar practice or service; (2) agrees to be Hylant Group’s successor in interest with regard to the maintenance and protection of the information collected; and agrees to the obligations of this privacy statement.
